Move patient intake from WordPress to FormDr
Keep your WordPress website while moving intake into FormDr. Plan the questions, staff workflow, and old submissions before replacing the form.
Does your front desk need a website login to find intake forms?
Your WordPress plugin may save every completed packet in its dashboard. Staff download attachments, search notification emails, and work out which version belongs in the patient record.
You want that work in FormDr. Replacing the whole website would be a detour. The useful project is moving intake while keeping the public pages.
Build the FormDr workflow before replacing the WordPress form.
Recreate a representative packet in the practice’s FormDr account. Confirm the agreement and settings, then test it with invented information. Have the people processing intake follow it all the way through.
Once they approve it, the WordPress page can send patients to the new form. The website stays, but the place where new answers are collected and reviewed changes.
Copying the questions is only part of the move.
Your form may include consent wording, conditional questions, signatures, uploads, and different alerts for different staff. Record those requirements first. Confirm what your FormDr plan supports rather than assuming every plugin feature has an equivalent.
Test the awkward cases: an incomplete packet, an attachment staff cannot open, or a patient stopping halfway through. Those cases can turn a neat demo into more front-desk work.
Uninstalling the plugin is not a records plan.
Old submissions may remain in WordPress, backups, downloaded files, or email. Identify the required records and compare any authorized export before retiring the tool.
Give staff a clear answer to “Where do I find last month’s intake?” The new form can work perfectly while historical records still need attention.

Before you decide
Should the form be embedded?
A clear intake button is a useful starting point because it gives patients one obvious route into the selected form service. An embedded form may suit the design, but it also needs testing with the website’s other scripts, on small screens, and for keyboard access.

Is WordPress always unsuitable?
WordPress is not automatically the wrong choice. Its suitability depends on the form plugin, hosting, agreements, settings, and how the practice manages the workflow. Moving forms is useful when it solves a specific problem, such as keeping intake records out of the website dashboard.
